CME’s Measurement Problem Is Becoming a Burden Problem

Simulation is losing novelty privilege. The stronger case now is objective fit, learner targeting, scalability, and credible proof of value.
Simulation is being judged less as premium technology and more as a format that has to justify where it fits, whom it serves, and how it scales. This is still a narrow, expert-heavy conversation rather than broad clinician demand, but across this week's sources the direction was consistent. A second, thinner theme extends the same discipline to course architecture: if retention matters, one-time exposure and immediate post-tests are a weak stopping point.
Across several simulation-focused conversations, the common point was straightforward: start with the learning objective, then decide whether simulation or VR is actually the right tool. One source stressed being explicit about what a modality does better than mannequins, simulated patients, or simpler formats before adopting VR (Medical Education Podcasts). Another emphasized matching simulation to learner level and local conditions rather than treating it as a uniform answer across settings (Faculty Feed). A third pushed the conversation further toward scalability, support, and measurable value (Simulcast).
This evidence is still concentrated in simulation and medical-education circles, not broad cross-specialty clinician conversation. Even so, the implication for providers is clear: simulation-based education now needs a more specific business case. Buyers will want to know what task it fits, which learners it is built for, what faculty and operational support it requires, and whether it can be deployed beyond a one-off showcase.
For CME teams, that means tightening product language. "Immersive" is weak positioning on its own. A stronger offer can explain the objective, learner segment, implementation model, and evidence plan before a buyer has to ask.
A separate but related conversation argued that exposure and immediate end-of-activity performance are poor proxies for durable learning. In that discussion, ACCME leadership and a learning-science expert pointed to spacing, retrieval practice, varied contexts, and multi-touch reinforcement as better ways to support retention and transfer (Coffee with Graham).
This is one expert-led source, so it should be read as a meaningful design cue rather than settled market demand. Still, it sharpens a practical issue for providers: many activities are built to end at completion. If the bar is moving toward what learners retain and can apply later, then the educational product includes the follow-up structure around the session. For CME operators, the decision is concrete: where are you still treating reinforcement as optional add-on content instead of part of the design? Even a light follow-up layer changes what you can credibly claim the activity is built to achieve.
