Simulation educators are testing custom GPT workflows that turn real-time transcripts and scenario rubrics into debrief outlines. The signal is narrow—a single journal-club discussion of pilot work—but it is a useful warning for CME teams considering AI-enabled simulation support.
In a July Simulcast journal-club discussion, simulation educators described a pilot in which a real-time audio transcript and scenario materials were fed into a custom GPT to generate a structured debrief outline for facilitators (Simulcast). The appeal was clear: novice debriefers face high cognitive load, and an AI-generated outline can make the next conversation feel more organized.
The caution was just as clear. The discussion raised concerns that the model could look neutral while quietly carrying the biases of its prompt. In the example discussed, the GPT output appeared to anchor on a rule embedded in the prompt even when learners had already reached the appropriate conclusion. The educators also noted that an audio-only workflow misses body language, eye movement, task execution, and psychomotor performance—the very details that often matter in simulation-based learning.
For CME providers, the implication is not “add AI to debriefing.” It is that AI-enabled simulation needs faculty development before it needs scale. We saw a related pattern in last week’s brief on simulation pathways: digital tools only help when human coaching, proficiency expectations, and feedback routines are explicit. This week’s signal moves that problem into the debrief room.
The operator question is simple: if an AI tool proposes the debrief agenda, who is accountable for checking what it overemphasized, what it missed, and whether the resulting conversation actually improved learning?
The same simulation discussion also touched on interprofessional code debriefing, where hierarchy, role clarity, and trust still shape whether co-debriefers can work well together. That matters here because AI does not remove the relational work of debriefing; it adds another participant whose role must be bounded. The stronger CME model is not AI-led debriefing.
