LLM Hallucinations Require Targeted Verification Drills, Not General Warnings

Educators are tying digital tools to simulation, protected time, and proficiency checks—not treating them as optional add-ons for procedural training.
Voluntary procedural simulation was described this week as producing only 20% compliance, while educators in physical-examination training named five ways digital tools can help convert practice into skill. The evidence is educator-led and still narrow, but the provider implication is clear: digital assets and simulation need to be designed as one required pathway, not offered as optional supplements.
The strongest signal came from two different kinds of procedural education. In robotic and minimally invasive surgical oncology training, a discussion of proficiency-based curricula described non-mandatory simulation compliance at 20%, rising to 100% when a two-week curriculum became mandatory with protected time before operating-room exposure (SurgOnc Today). In physical-examination education, clinical educators described digital tools supporting five functions: sensate knowing, modelling, rehearsing, guiding practice, and feedback (Medical Education Podcasts).
Those examples are not the same specialty or learner level, which is the point. The common thread is not robotics, ultrasound, tablets, VR, or mannequins. It is the sequence: prime the learner, rehearse the task, guide the attempt, give feedback, and require proficiency before higher-stakes practice. That extends the ability-based progression pattern we covered in an earlier brief on replacing time-based CME with ability-based progression, but makes the operational burden more explicit.
Debriefing is part of the same infrastructure. A simulation journal club discussion focused on how instruction inside simulation is often under-specified, including the role of scaffolding, human tutors, computer-supported prompts, pauses, and near-real-time feedback (Simulcast). For CME providers, that means a simulation activity cannot be judged only by fidelity or attendance. It needs a documented instructional model: what the learner must do before live practice, what faculty observe, when support is withdrawn, and which performance measures decide readiness.
The concrete question for CME teams: where in your procedural learning portfolio is simulation still voluntary, and what would have to change—schedule, faculty time, budget, or outcomes plan—to make proficiency required rather than encouraged?
The next test will be whether AI-supported prompts become a disciplined part of debriefing or a shortcut around faculty judgment. A clinician note this week argued that learners may still be better served by mentors or peers than generated output when the tool adds too much verification work (X). CME teams do not need to reject AI-assisted debriefing, but they should require human review, prompt logs, and clear override authority before treating it as instructional infrastructure. If blended simulation pathways keep moving in this direction, the differentiator will not be who owns the most impressive equipment. It will be who can prove that the equipment, faculty guidance, and assessment rules reliably move learners from exposure to demonstrated skill.
